OP here. I bit the bullet and waited 6 months to deactivate online on the very day I was allowed to do so. A few days before, I chatted with a Sony rep to make sure that I would be able to deactivate on that day and that there were no problems on their end. I was able to finally activate the only PS4 I ever owned as primary. Not sure if it was a hack or not, but I'd like to think I never gave the culprit a chance to re-activate their PS4 if it was, and I did change my password for good measure anyways.
It really does stink though. Without a primary PS4, I couldn't make use of remote play for PC. I couldn't make use of automatic uploads for my PS plus account. And if I had a PS Vue account, I wouldn't have been able to use the DVR abilities.
