Care to elaborate? I didn't really get into that game because the combat felt really wierds. Like trying to write with your left hand when you're a righty.

Fallout New Vegas.  It's a realistic mode that they are instituting to make it more funnerer.

I don't find realism all that fun.
 
If i wanted to kill a Radroach realistically I'd start a nuclear war then go out and kill one. None of this pretend shit.

Off of the top of my head:
You have to drink water
Bullets have weight
Med-kits aren't instant

Fuck, another mode which I'll hate playing, but feel like a pansy by not playing. Not to mention the inevitable achievements I'll feel obliged to...achieve.

According to the Giant Bomb page for Fallout: New Vegas

Hardcore Mode

 
One of the new features announced for Fallout: New Vegas is the new difficulty setting called 'Hardcore Mode'. The mode can be toggled on and off at any time in-game. In Hardcore Mode the player must regularly drink water to remain hydrated, ammo will have weight, and stimpaks & radaway heal over time as opposed to healing instantly in other modes. Also, stimpaks cannot be used to heal crippled limbs. The only way to treat crippled limbs in hardcore mode is to visit a doctor. The enemies also behave more intelligently in hardcore mode. It is also possible to heal crippled limbs using a doctor's bag.  
 
 I'll definitely try it out. It adds some nice depth to the combat and travel.

basically it makes the whole role playing aspect real. If you don't regularly eat or drink, you'll eventually die, stimpaks and radaway heal like real medicine. And hardcore mode can be turned on or off
