Johnny, also known as Akiba in MGS4 is actually a character who has been around since the first Metal Gear Solid. In Metal Gear Solid and Metal Gear Solid 2 Johnny is a bumbling guard who serves as comic relief when he has his uniform stolen by Meryl and when he gets diarrhea when he's supposed to be guarding a captured Snake's prison cell. However, in MGS4 he appears as a member of Rat Patrol, Meryl's squad sent to observe and keep peace in various war zones. He's more useful in this game but not much less goofy as he continues to contract diarrhea and doing silly/dangerous things throughout the game.
Later in the game he saves Meryl's life. Upon removing his mask, it is revealed he looks a lot like Snake in his younger days. From then on, he becomes a love interest of Meryl's.
Johnny was also briefly heard in Metal Gear Solid 2: Sons of Liberty on two occasions. The first is after you find Richard Ames. While listening to Solidus and Ocelot talk, you can point the directional microphone to the left of them where you can hear Johnny sitting on the toilet talking to himself about Meryl and Shadow Moses. The second is when you are helping Emma Emmerich get across the connection bridge. When Emma goes behind the middle pillar the player could take out his directional microphone and listen into a humorous conversation between her and Johnny, which ends with him getting diarrhea and running away.
In Metal Gear Solid 3: Snake Eater , which takes place during the 60's, a great deal of Johnny's origins is revealed. During a hidden cutscene, while Naked Snake is in prison, the solider who is guarding Snakes cell is revealed to be one of Johnny's forefathers. He has a son named Johnny and later states that all of the first born sons in his family are named Johnny, including himself.
Johnny's forefather also suffered from a bad stomach, although in an amusing inversion, while Johnny suffers from recurring diarrhea, his ancestor was plagued by constipation.
