So Jack died.
And man... I was not happy with that.
She just got shot because a ship got attacked?
Really?
It was the most depressing thing ever. It's not like she was fighting or anything. She died INSIDE OF THE SHIP!!!!
And everybody else survived.
... Man... It really bummed me down and I wanted to restart the game but that's gonna another 24 hours(my total play hour was 25 hours)

So does everybody survive if everybody's loyalty is high than normal?

I know what you mean dude, Jack died on my run because of the fight between her and Miranda, I wasn't 75% Paragon or Renegade so one of them had to not trust me anymore. Really sucked :/
Frpm what I can tell, you can do everything "right", and still have one of the weaker characters die, esp jack and mordin. I did the first time, played again with the exact same choices, and everybody lived that time....

Either send Mordin back with the crew, Take him with you, or leave all of the heavy hitters (Grunt, Garrus, Zaeed) to hold the line and be quick about finishing off the terminator. Your speed at the end seems to have an effect on if people live through that fight, as well as who's down there. Mordin is almost guaranteed to die if you leave him there, followed by I think Tali, Kasumi, Jack, Miranda, Jacob and so on. This is assuming that they are all loyal, if they're not loyal they are not making it out, end of story.
I was lucky not to lose anyone. Though the loss of Jack doesn't really bother me at all. She was an annoying character with way too many issues to be bothered with. Hence why it got to the point where all she'll say to me on board the ship is F#$% Off.

I guess I'm just that awesome of a commander! XD "I wrote it out wrong, the order that I had assumed everyone was loyal, the timer take out none loyal first.
I was also exaggerating the time you needed, I only lost one of my non loyal during my Insanity run, which I didn't expect :)
Hehehe... this topic is funny. Garrus is pretty ugly, but no one wants to kill him off because it's okay for dudes to be ugly.
Also, keep your save where characters die and it hits you on a personal level. If you go back and reload a save, it will destroy any verisimilitude the game has and drastically reduce the effect the story has on you. I know it sucks, but it's truly for the best.
