Look through the 'Scene Select' on the Main Menu and make sure all the areas were cleared on Realistic. I'm pretty sure the game bumped my difficulty down at one point without me noticing, but whether it was a weird restart glitch or because I was failing an area multiple times, I don't know.

Go back when you have the Abyssal Chain. There's a Shadow Current in the north part of the desert that you can reach after you have Ruin, and some hooks that you grapple between to ultimately turn off the drill.

I know there's a way out, but without playing through it right now I may be vague. In the room with the angels on pedestals--I think--one of the ways out ends up leading through a hallway with lava flowing, where you have to jump over and climb over a bit, and ultimately leads to the spot where you pushed the first statue over the edge to create the hole. There's demon creep you can climb up that leads to the courtyard.
