Yeah, I got that the wife is in the arm.
Right at the end, in his memory, she says: "I'll always be at your side."
And near the end, when you hack a terminal-thingy, you find a report that states how Spencer sat there thinking of his wife, and also sat there, staring at his missing arm, both with the same sad look.
 
I'm guessing that he subconsciously knew she was in there.
