RPGs which end when your lead character dies. Who thought this was a good idea?
Things that make me put the controller down
Any game where before the final boss, they throw every previous boss at you in some final fuck you gauntlet.
The first time I killed someone on MW multiplayer, picked up their weapon, and discovered they had a heartbeat sensor/thermal scope.

Homonucleus or a null-light/dark pokemondemonthing. If the game doesn't give you the materials to evade it, I agree, its stupid.
But if the game does then, I don't really mind(well, I do. I just mind it less and don't think its bad design).

I love Nocturne, but when I get hit with a death spell, I have to walk away for a bit. It feels like the game just stole part of my day. I'll concede that some people enjoy the challenge, but it's not for me.
A lot of what others have said.
-Crappy checkpoints (whether it's the system, or I got really unlucky and managed to checkpoint as a grenade/rocket/etc went off next to me).
-Getting spawn killed repeatedly because my team doesn't seem to care that there are three guys in our spawn area.
-Missing any time limit by a millisecond, or really crappy timed missions.
-Any beat-related mission in a non-beat related game ( Fuck You "Life's A Beach/Lowrider" in San Andreas. Fuck you. TBoGT's dancing was fine though).

-Really crappy friendly AI. This one won't make me stop playing or anything, I'll usually just sigh and keep playing. I might even go out of my way to save as many as I can (I always try to save as many friendlies in Halo as possible, not sure why, but I do).
-Losing a lot of data (admittedly it's mostly my fault not saving, but still...crashes or power outages can piss me off).
Most of these I'll just put down the controller for a few seconds and calm down before playing again. But stuff like that 'losing a bunch of data' one, I'll just quit and sit there mindlessly staring at the screen for a bit before watching some TV and leaving the game alone for a few days.
Edit: Oh god, I forgot a big one. Crappy escort missions. They have low health, or don't fight, or the enemies only focus on them. Crap like that. Some are okay, few are pretty cool. The only one I can really recall though is the second Agency mission in Just Cause 2. You escort the dude by standing on the roof of his car and shooting dudes who follow you. Now that is how you do escort missions.
When there's something of more interest not in the video game, but somewhere else in the area or outside or something...
Technically, this is the only correct answer.
I wouldn't say there's any one thing which is enough to make me just put the controller down on its own but there are a few things I find generally rather unpleasant in games: escort missions, timed missions, invisible world barriers which kill you when you go beyond them, very poor camera, serious glitches.
