As someone who has beaten Insanity I can honestly say that that is simply not true. I have to say that I beat it just the way I described on insanity.  The "big" boss is not that hard.  I tihnk the game would have ended alot better with a Gears 1 General raam, that requires you to move between cover constantly. "
Maybe because I was playing as an Adept? "

Could be, ME2 heavily favours soldiers.

Dude what, all you had to do was hide and pop up and shoot his eyes.  It was one of the easier fights in the game period

Probably something ridiculous like Mario.  Jacking into a universe like GTA or Mass Effect wouldn't mean anything, you would just be a regular guy in that universe.  Mario is so messed up just being there would be crazy.

Ryan wasn't saying that it was wrong that the formula was the same just for the sake of it, he was saying that janky aspects of the formula that have been in the series from number 1 are still there.  Watch the quicklook, some of the camera style issues (falling off the bridge) are a staple of the series.  While third person action games (such as Batman:AA) have come along and brought new and more involved/compelling gameplay style, GOW has largely remained the same.  There are people out there who don't care at all, however, and like the game just the way it is.  You are one of those people, so just don't worry about it and have fun playing the game.
