Firm but not so hard that it seems like you're trying. It's best to leave the impression "you couldn't get out of this grip but if I really wanted I could break every bone in your hand".
Depends, usually firm but not crazy. If someone comes at me with the death grip they get it back in spades and regret it. My grip strength is disproportionately stronger than the rest of me even though I play football/am a big fellow.
I had the "Firm handshake while looking into whoever's hand your shaking's eyes" concept drilled into me as a kid. I've always felt that making a good first impression starts at the handshake (as does the assertion of dominance).
Medium-Aggressive. It's funny when the gangster kids I meet try to give me daps and I just end up crushing their hand and shaking it. They're always dumbfounded, like they've never met an adult before.

Yes, and to assert your dominance further, make sure your palm is facing somewhat toward the floor so theirs is facing somewhat up (in the submissive posture).
A whole lot happens in the first few moments of meeting someone, even if you aren't aware of it.
Hold about bringing the clasped handshake real close to your crotch while looking them in the eye is that a good idea. Just like a few inches away from the partyzone, fly down + unbuttoned -- casual Friday style as it were. That asserts dominance and interest.
Firm though I've been told a little too tightly by a few people, like I was trying a little too hard. That usually only happens when I judge someone that I feel would require a firmer handshake than usual though; apparently I'm usually wrong. :/

A little too firm. I always handshake my friends and we try to crush each others grip, i end up inadvertently doing it to people i just meet. In freshman year of college I met a friends BF and gripped his hand as hard as I fucking could without even realizing what I was doing and he was like "dude what the fuck?" We never spoke again after that
