Here in Cork, Ireland we have a few.
I'd recommend Lennox's on the Bandon Road or Looney's in Mayfield.

If you're in Madison, Wisconsin, head down to the capitol square and find a restaurant called The Old Fashioned. Find this on the menu, order it, eat it, and let your eyes roll back into your head:

If you're ever in Regina, Saskatchewan, you're probably gonna have to go to Burger Baron. Their mushroom burger with bacon and swiss definitely drops a bomb on it! BOOOOOOOOOOOOOM!
IDK if they have the best burgers in Florence, AL., but Montagu's has amazing burgers and philly cheese steak sandwiches. I go there over fast food anyday, because they are down the street, and I have drinks at home!
There's a 5 Guys that just moved in down the street from me, but I've yet to try it. According to this thread, I'm making a huge mistake! I apologize and I'll correct it immediately.
Until I do that though and if you're in Houston, TX, go check out The Itty Bitty Burger Barn. It's a-mazing.
Seattle has a lot of great places, Red Mill is fantastic, Dicks are great (and shakes, come on, who doesn't love shakes???), 5 Guys, and plenty of other places. I'd say Dicks because the meal is over all the best, but Red Mill best for burgers.
In both the Buffalo and the Rochester Regions of NYS There's this semi-local chain of pubs called MacGregors. In addition to having a fantastic beer list (I don't particularly care for beer but I'm still impressed by the sheer quantity of local brews) they have their take on the most awesome burger ever, http://cheeseandburger.com/havana

ohhhhhhhhhhhhhhhhhhhhhhhhhh man, I just clicked on the Havana....I shouldn't have. That thing looks outstanding. I actually have a friend studying pre-med in Rochester, me thinks its time I pay her a visit!
My house, As i make burgers how I would want them served. Multiple patties, multiple types of cheese, The leanest beef available mixed with some Fatty pork to add that extra grease and flavor. Season the shit outta it with my special blend. I'm hungry. Or Uncle Dave's if your near kitsap county in Western Washington. Not the best I have had but they are the only place thats not a chain that gives 2 fucks about what you want and not how much money they can get from ya.

5 guys is good...but In N' Out trumps all burger chains. Incredible food and value. Fries cut right there in front of you. Everything is super clean. Great service.
Get to Bath, try a Schwartz burger. Remains the best burger I have ever eaten.
My kitchen. I actually don't know of any good non-fastfood burger joints in Petaluma. I haven't actively looked though.
Well, in Salt Lake, you have options. The Training Table is a local chain that is really good, though not my absolute favorite. Another is Salt City Burger Co., which I really like because, in addition to making great burgers, you can pretty much put anything you like on it. The best is this place called Hires Big H, which I just really like. They have great root beer.
I see someone chimed in from Rocky River, but let me chime in for greater Cleveland. Cleveland is the home of Iron Chef Michael Symon, who I think just won the South Beach Food and Wine festival burger competition for the 3rd year running. He's opened up B-Spots in 3 suburbs in Westlake, Woodmere, and Strongsville, with a 4th coming to the Horseshoe casino when that opens downtown, and a satelite at Quicken Loans arena. The burgers are fantastic. My personal favorite is the Fat Doug(beef, pastrami, swiss, slaw and stadium mustard. My wife favors the Lola burger(beef, bacon, fried egg, cheddar and mayo), or the shroomage(beef, a giant portabella, blue cheese, grilled onions, and the house's steak sauce). Don't forget sides though. I swear by the Lola fries and Siracha wings. And remember to wash it down with a vanilla bean apple pie bacon shake.
Ok if you are in Frederick, MD and have to eat a burger, don't give in to the pressure to go to the Five Guys, go either to Beef 'n Buns 'n Paradise or Freez King. Both aren't chains, and serve rather amazing burgers. Freez King is a long time ice cream joint, but closed a couple years back and re-opened as a burger n shake joint. Still good. Beef 'n Buns 'n Paradise used to be known as "Cheesburger in Paradise" but Buffet is such a dick he went around suing all the mom and pop joints trying to make a dollar (see the irony, both ways I do) that they had to change the name. Order a Frenny Burger w Fries and a shake, or a Double Frenny Burger if you want to die (with fries and shake) and enjoy!
If you're ever in Dublin, Captain Americas does some crazy big burgers. But if you want more traditional chipper type burgers then Leo Burdocks is the place to go!
Albuquerque NM, Bob's Burgers is a good place, get a #1 Ranchero Burger with red or green and cheese, chili cheese fries to match, and a bottle of Tums. Taco burger tuesdays is also an acceptable choice from Bob's
Other acceptable places are Blake's (Green Chile & Cheese lotta combo), 5 guys cause its good everywhere, Lumpy's, or Little Red Hamburger Hut.
