My shepard is worse than that, and I did it on purpose. 
 
As for Miranda once you do her Loyalty quest you'll never look at her face again.

I picked Warp Ammo, just straight damage boost, does a good job for me.

This, Vanguard is pointless on easier difficulties, where as Adept is really fun when you're dealing with enemies that have no shields/barriers/armor.

@h83r:  As long as you have gotten Jack's loyalty at some point it is available from the start. It doesn't need to be on that run.

THAT. was your first mistake. Vanguards are close quarters oriented. On insantiy, close, is the very last thing you want to do.   My advise: Play through the game a few times, know what to expect. Have multiple save files, those missions the Illusive Man sends you on that you can't avoid can kill you, so have a save you can revert back to if things go sour. Import a ME2 character at level 30. Particle beam does wonders against Preatorians, whereas the Arc Projector will save your life on the Derelict Reaper. Other than that, take it slow, abuse cover, save often, abuse powers, and do loyalty missions to get the upgrades on them. Vanguard is not a mistake. You have to be close up yes but they have the skills to do it. Get an Evicerator or Claymore (Evicerator is my preference) stack health and shields on your armor and be mindful of your position, so watch for cover and furthermore, Stay the hell away from Harbinger. He picked the route that requires the most skill. 
 
For Vanguard take Miranda as a requirement. Others to take are Thane because he uses a sniper and can use throw. Combine it with pull and have him attack anything you can't reach with Charge. Zaeed or Garrus depending on shields or Armor. My personal favorite is Grunt just because he can take a friggin' beating! Tali's drone is good but there isn't much else for her against the Collectors. 
 
The Collector's Ship is without a doubt the hardest part for a Vanguard.  Rest of the game is easy after that.

Get to the collectors ship, take a sniper, cast singularity and snipe down armors while its in transit. Singularity is slow and when cast from a distance will give you time to line up shots.

Haha ok, let me explain. 
 
Start a fresh game, and get to the part where you find Jacob, He only has a pistol with him just like you. Now save and restart the console. When you reload the game, he will have a shotgun with him (Evicerator is the shotgun you get from dlc)

So I just started up my adept today, got Jacob, then I had to restart my router, kicking me from live, starting a profile change error, sending me back to the title. So I just shrugged and reloaded the game, look to my left and Jacob is holding an Eviserator in my face. 
 
It's obviously only an assist for the first 5-15 minutes of the game, but hey if you can give him a free shotgun from the start why not right?
