Then there is also the fact that you are probably in a higher level dungeon. I found myself struggling a few times in areas I knew I shouldn't be in yet.

I wish there was a perk that gave a % chance to impale onto any surface nearby, or at least to decapitate. Something to add to the enjoyment I get from one-shotting people between the eyes.
