Someone go read Quartette and tell me how it is. Given its nontraditional setting and art, sounds like its right up the alley for some of the guys in this thread. (No idea on the quality, hence the "tell me how it is" bit, though it doesn't look likes it going to go the tentacle rape route.)

Let's Talk - How Can We Make Eroge (Hentai) Popular Outside Japan
The only way these games will have a chance is if anime and cartoons are not just perceived as a child's thing. The reason it sells over in Japan is because anime is so ingrained in the culture, business men read Shonen Jump for god sakes.
With that said there's really no way, it's just the culture. It will always be a niche little hobby that you have and you won't want to tell anyone because you'll be shunned by them for the rest of your life.
Unless of course Japan takes over the world, which will happen as soon as they make Gundams real.

Following up on what I said earlier, some cultural differences just won't fly outside of Japan. If you break down the basic tropes of a typical Eroge game, you end up with only a handful of character types:
The Mother: An older, experienced woman. This is usually a teacher, or someone in a position of authority. In a lot of cases, it's an actual step-mother figure, so that an extra level of taboo can be injected into the scenario.
The Sister: Or the Girl Next Door. This would be the most common; a character of equal age to the player, usually a class-mate or co-worker. Again, actual step-sisters more often than not fill this role.
The Lolita: This, by far, is the most contentious character type, and one that, unfortunately, has become almost ubiquitous within the genre.
The first two tropes are something that we often see in western pornography, so nothing new there.
But the third, the Lolicon, is just not acceptable when taken outside the confines of Japanese culture. Even in Japan, it's become an issue of debate. To even attempt to understand it, you have to look at the evolution of Japanese sexual culture as a whole; something I won't go into here, since I'm not sufficiently qualified to lecture on the subject.
In the end, this is the biggest stumbling block to Eroge in the west (though it has to be said, even in mainstream titles the use of the Lolicon figure is something that can result in changes to content), and until developers can shake free of this particular albatross, Eroge will stay the domain of male thirty-something social lepers, who marry love pillows and think that little girls are something dreamt up by Nobakov.
