Decided I could run past the zombies in a dark and narrow hallway in Arrange Mode for Resident Evil: Director's Cut.

Flamed, machine gunned and missiled to death by Sentrybots, Protectrons and Turrets.

Something in Super Meat Boy.

Got shot up a lot in Mafia 2

I think a large tree killed me in Fable 2.

killed by a guard for not paying my bounty in Oblivion.
 
loaded up the quick save then killed guard.

Got shot by Vengeful Hunter in DotA.

Somewhere in the Rapture section of Super Meat Boy....well, I guess not so much somewhere as EVERYWHERE!!!!!!!

I was playing Vanquish, when a Dr. Manhattan looking RI appeared. Apparently its called Assassin RI. Got my ass kicked, most likely because Im playing in Hard though.....

I would love to say 'eaten by zombies in RDR', but sadly that's not what killed me last.
I've been killed by a tarp placed in Irenicus' dungeon. Eff that guy. Seriously.

Death by an electric floor in Limbo.

Crashed my fucking car in Deadly Premonition, went from 20% durability to 0% in one crash. Before that I was decapitated by the Horseless Headless Horsemann in TF2.

Was a section in Castlevania: LoS in a castle... :(

Playing Diablo II, trying to level up a Sorceress build I've been trying to pump up for a bit. Got surrounded in Act II by a horde of beetles who emanate electricity when damaged. A lose-lose situation. 

I was shot in the back by another survivor on the roof of a house in Gaptooth Breach while I was sniping zombies. That little wanker shot me twice in the back. -_-

Three Cesar's Legion assassins showed up and blew my limbs clean off my torso. :(

Those stupid giant moth things with orange wings kept killing me while I was trying to climb a mountain to get to a camp in New Vegas.

then i fast traveled and the game froze so i turned it off and sent it back from the rental place.

5 minutes ago, got killed in the Adventures of Link by the boss of castle 3. He's the blue knight on the horse. Went into the battle low on health and magic and on my last life, could only take one hit. I took out his horse form, but his knight form killed me.

Some how let a group of zambies pull me off of my horse and watched as the fuckers road off with it when I got killed by something spitting out this green stuff.

NCR didn't enjoy the fact I opened a door that wasn't marked as red. So they shot me, lots...in the face. 

Was about to go restart the generator in The Sacrifice, and I did, got the achivement, but then it didn't count because all 3 members died at the exact same time the ending started.

About 20 mins ago...got my head lopped off by the horseless headless horseman in TF2.

Damnable zombie horse threw us both into a ravine.

Mordekaizer ulted me.

I thought I could kill Master Yi before the tower killed me.  I was wrong, and Yi got first blood.

Haha!  This is a great topic, very Halloween orientated.  Anyways, my last death was playing Resident Evil 4 on the Wii over the weekend!  I just backed myself into a corner by a bunch of "not-zombies" and then a chainsaw dude buzzed me in half because I had nowhere to run. :(

I was overrun and eaten by zombies in Red Dead Undead Nightmare.

As a Heavy killed by a beautiful headshot from the Blu team sniper!  (I was wearing a pumpkin on my head and hunting for the Heavy Bag Mask)

I just couldn't get that ever so needed line block.

Swan dived into stone. Apparently sometimes Ezio doesn't feel like landing in safe material. Before that, I died via Banshee + Fuel Rod Turret combo in front of Sword base in 4-player co-op.

I jumped off a ledge into an abyss, trying to mount a panicked Yoshi.

got mauled by a sudden ninja zombie cougar attack in RDR, damn ninja cougars...

Was surprised by a Powder Ganger in NV and was blown up with dynamite...

Mean pirates scrambled my engines and blew up my 50 million credit battlecruiser in EVE : Online.  Low sec space is scary.

Was followed by a huge ass spider in New Vegas and got killed in 5 seconds by it. That little fucker.

I ran out of Haven holding a small baby with Raiders firing at me. I got slaughtered.

A Ghost Pirate hit me with a broom.
