The worst game you ever rented
Would probably be The Davinci Code: The Game. That's my worst game ever so far. GB doesn't even have a page on it, not that they should. I also remember Predator: Concrete Jungle being pretty bad.
Back in the day it could be a minefield. I mean, obviously there's still crap games but they're a lot easier to spot now. I'd have to say it's between The Terminator for the SNES and Ninja Kid or The Legend of Kage for NES.
I was at a gamestop the last weekend because I had gotten a gift card a while ago and was going to use it to get Stalker, and while I was in there I checked out the few PC games they had in the store and what jumped out at me was Pariah. I remember it probably about as fondly as you, what struck me was the fact that those fools were peddling that crap for $45. It was crazy.
Anyway the worst game I've ever rented has to be Onechambara: Bikini Samurai Squad. That was the single most terribly awfully horrifically bad game I have ever played.... Except for maybe ET on atari.
Most recently, I rented Pure a while ago. Played it for an hour and then never picked it back up. Seemed like it was trying too hard to be SSX with ATVS and failing.
I stopped renting games after the SNES era, but the only one that comes to mind was Fester's Quest for the NES. I'm sure that I got worse games, but I've blocked them from my memory. I honestly don't even remember if Fester's Quest was that bad, but I don't think I even played past the first level.

Prince of Persia the sands of time, one of my favorite games on the PS2. It is the only game I have rented so it is the worst and best game I have rented.
Rayman Raving Rabbits on Xbox. I had some cousins come in from out of state so my brother and I thought that it might be a good party game.....we were wrong, dead wrong.
Yeah, I have no idea why I rented this either.
Having only owned a N64 and a Gamecube when I was into renting games, I was victim to a bunch of terrible titles. However, I think Universal Studios Theme Park Adventure. Wins the "Award" of worst game I rented.
