Hmm... one of my friends linked me to an Islamic beheading video. It was fucking brutal. Oh, and the Encyclopedia Dramatica 'Offended' page... dear God. Well, I'm not getting to sleep now tonight.

No clue why, me and my friends never knew anything was wrong so we just went there to hang out like we usually did. 
if only we had gotten there earlier.

Not really, but ever since that was brought up in some twilight-related thread, I always end up thinking about it.
 
Seriously though, I would say Knife Fighting a bear is brutal. Especially if the bear also has a knife.

oh yeah, i remember! Driving down the highway, there what a car wreck, with people burning alive inside. I guess one was still alive as he clawed his way out of the flaming vehicle whilst burning to death. that was fucking brutal!

I'll add something useful, I have a third degree boiling water burn down my leg,  A nice scar, 
 
And I got it from a butchery... 
 
I saw my boiled skin fall off my legs, just like a sausage. 
 
I can't go into butcheries without having flashbacks of my burn, 
 
The smell, brings back the pain.

So many stories in this thread so far that I could perfectly picture Nathan Explosion watching, staring, and then just going "That was brutal." afterward.

a kid in 8th grade was waving to his "girlfriend" across the street as he was crossing, then a pick up hits him and sends him flying across the intersection

It's weird, I never really watched Metalocalypse until the end of the last season. I never really cared about it for all the time it's been on air, but then I watched an episode. Then a few more. Then it was brutal.
 
The ending of last season definitely was brutal as all hell though.

A puppy being run over by a metro... I couldn't do a thing. 

 
Couple things.  Saw a guy on a motorcycle speeding down a neighborhood road.  Smack into a car and roll over it.  The biker and the bike.  Then the guy stood up and drove away.   While this girls dad ran out of his house and tried to stop the guy from driving away.  By grabbing his shirt.  But the crackhead on the stolen bike punched  him in the face as he drove off.  My friend and I just stood there in shock. 
 
Once I tripped over trying to pick up my toy gun(It made sparks!) and stabbed myself in the left eye.  With a shelf holder.  Stuck almost 2 inches into my eye, before I flinched and ripped it out.  I was about 10 or so and had to have the ambulance come and take me to the hospital.  Where I had emergency surgery and almost lost my eye sight in my left eye.  Hurt like hell and had to wear a eye patch for weeks.   
 
Also got into a fight in school once.  As the teachers were breaking us up, the other kid grabbed a piece of plastic.  Flung it at my face and gashed my cheek open. Had to get stitches and the doctor let me see my skull(cheek bone) before he stitched me up.  Looking back it was pretty fucked up.
 
Those internet shock videos are brutal too.  I'd stay away from them if I were you or at least don't stay up late watching them.  Then go to sleep.  Dreaming of those videos and GIFs is awful. 
 
Edit:  3 guys 1 hammer isn't the islamic beheading?  I thought those were with machetes or the one with a knife.  You know the GIF or video of the guy on top the other tied up guy.  Guy on top grabs the other's head pulls up, exposing neck.... The saws his head off.  Not chops off, SAWS!  Blood everywhere, guy squirms, head tears off.     

Um since we're big on the dead bodies, when I worked as a primary care paramedic me and my partner answered a call at a motocross event where a journalist had been cut right in half by a bike which went off the track. Don't ask me how (I was told the chain detached with such force that it cut the guy in half), but it was pretty "brutal" I guess.
