Note: For films, I usually put 'subtitles for hard of hearing' because I love seeing, *MAN BANGS DRUM* and such like at the bottom of my screen.

I do for games, mainly because it can often be lost amid all the noise of gameplay (and partially because most games still don't really to lip-synching well enough (in my opinion anyway) to really aid in the understanding of what is going on).
Usually always, unless it's in the middle of the day and i can wang up the TV volume.

To further prove my point:
All threads made by this guy that are on the front page.
Only and always in games. There is always something important to the plot or gameplay that needs to be absorbed. Its all to often that a character's accent, bad voice acting or something else distorst the lines. Subs let me take the info. in on two levels. So I am far less likely to miss something.
I hate subtitles, they confuse my concentration and I can't really enjoy the movie/cut scene. The girlfriend is used to read subtitles (english is the relationship language) so I usually have to endure having them at the bottom things.
In movies whenever the voice audio is too low or the accent is strong. In games all the time if there is the option for it.
It really depends to be very honest:
Movies: If I'm watching a movie where I don't know the spoken language, then Yes.
Games: If the game is very action oriented and is very heavy when it comes to static/background-noise, then yes. Because I don't really like to turn off the volume on the sound effects because then I loose a little of the experience.
Only in games whenever available. I'm playing Grand Theft Auto IV currently, and subtitles make it way easier to understand what Little Jacob is saying.
