To be different, I'll say Far's version of "Monkey Gone to Heaven" and Saul Williams's cover of U2's "Sunday Bloody Sunday". I say that they don't outdo the original source material, but do a good job making it somewhat different from the original material, and also making it totally listenable.
But of course, the greatest cover ever is Nine Inch Nails' "Hurt" covered by Johnny Cash, as mentioned here many times. It blows my mind how freaking melancholy and powerful that song is, especially compared to an already messed-up song (in the "good", emotional way) like "Hurt". It's to the extent where Johnny Cash's version is better than Trent Reznor's original interpretation of the song, which truth be told, is quite challenging. Also, I vastly prefer the softer, piano version of Mad World that's seen in that Gears of War trailer than the original Tears for Fears one.
By the way, thank you Nick for reminding me how awesome Weezer's version of Velouria is, and thank you Meltbrain for introducing me to TV on the Radio's cover of Mr. Grieves. It's easily another of one of my favorite covers now.
Johnny Cah's Hurt is definetly up there for me, but this one is also a personal favorite:
