I need them. I need all of them.
You know what I'm talking about: Not skeletons. Not skull masks. Fleshy bodies with fleshless heads. Skull heads.
Can you dig it?

@Hamst3r: Not quite sure how you'd qualify this one: Kid Chameleon
His real life manifestation is that of a basic kid, but in the game he can pick up a skull helmet and turn into a tank-driving-skull-headed tank driver guy. You can see it on the cover:
If I remember correctly, for a few seconds in between transforming into that tank driver, his head is that of a skull, but his body remains fleshy.
I was at the end of a week long meth binge and started picking at my face. You know how it is when bugs burrow into your flesh and you just have to dig them out with a rusty coat hanger you heat up with a lighter. I was shocked to find there was a fucking skull under my scalp. That means that inside us all is a stalfos waiting to break out and murder people. WTFuck man!!! That is some spooky shit.

@JCTango: Man I haven't thought of that game in years.
It's one of my love/hate games on the Genesis - I love being able to swap abilities based on what I want to do (and what's available), but it's also pretty dang tough. Plus those bodiless boss heads with eyes that pop out when you kill them freaked me out too as a kid.
I play it again once in a while.

Question here is if human characters that transform into skull headed humanoids should be added to this concept page. I'd say yes, but could go either way.

Skull-headed humanoid is the key. If they're fleshy everywhere aside from the head, they're good to go. If they're bones all over, they're a skeleton.
Also, in regards to the transformation aspect: magical transformations are perfectly fine. If they just put on a costume, helmet or such, that's not really in the same spirit.
EDIT:
@Video_Game_King That Mother Harlot one is great. I wouldn't have thought of her.
Death from Darksiders 2. He looks pretty fleshy?

I could be wrong, but I got the strong impression he's wearing a mask.
I have never even considered this idea before. Now that it's been brought up as a concept... I now want to know all of them. ALL of them!
Though it's not really used anymore, the old design for Tezcatlipoca totally fits the bill.
I had a question about this that didn't seem like it warranted a new thread. Does their body need to be human or can I add an animal with a skull head?
