Trying to play a conjuration focused Dark Elf in Skyrim on the master difficulty. Sent out a Flame Atronach to fight two Spriggins but one of them tagged me in the back and was draining my health. I'm only level 5 right now so I can't put up much of a fight myself and couldn't get the healing spell equipped and running quick enough to stop me from dying.

Beat to death by a pipe being held by the G-Virus infected William Birkin while stuck between him and a wall in Resident Evil: Operation Raccoon City.

OH IT FUCKING HAPPENED AGAIN! I DIED A SECOND AFTER COMPLETING ANOTHER MISSION AND DIDN'T GET THE CUTSCENE AND THIS IS AFTER I HAD TO REPLAY THE SAME WHOLE GODDAMN MISSION TWICE LAST NIGHT WHEN I FAILED BOTH TIMES EVEN THOUGH I'D REACHED THE DESTINATION AND TRIGGERED THE END DIALOGUE FOR IT, ONLY FOR SOMETHING TO BLOW UP ON ME THERE. (it wasn't a checkpointed mission) FUCK YOU SAINTS ROW 2!

Woo, ok I'm better now. But seriously this is bullshit. Although I will admit this time I have mostly myself to blame as I could've fallen back and healed a bit rather than standing in the middle of a goddamn crossfire, or learned how to properly use a healing item (I don't know how, and that would help I'm sure) I just figured out how to use a healing item and I'm a dumbass. Ooooh is that a tag spot? I'ma go spray that bitch.

Boss shit on my face in Diablo 3

In one of many tasks assigned that mission to me and my 3 buddies we had to hack a database.

It was all going swimmingly until I got stabbed by a Phantom, bitch.

I was cornered by two Forsworn Pillagers and just wasn't paying attention to my health (as always). Archery at short range is rarely a good time.

Died to a horde of zombies in Nation Red's Free Play mode after some long and exhausting (but fun!) co-op goodness :)

Queen Liga I of Lithuania died of pneumonia at the age of 40 and sent the entire REALM into disarray!

I got killed by a missile while taking a joyride.

Turned a corner and got my face shaved off by a shotgun wielding Bulldozer (Juggernaut) in Payday: The Heist. Panic Room is a bitch.

The cops blew me to bits walking out of the jewelry store at the start of Reservoir Dogs.

I got horribly abused by this drago..dracoloth in Tera. He did not want to share the surrounding plant life with me.

For me it would be Diablo 3 but i played a little witcher 2 before i came on here and got surrrounded by nekkers

Enemies with the Molten effect leave behind an orb that will explode after a second.
I've died 5 times because I either went to loot or took a breather after winning the fights.

Switched from my Barbarian to my Witch Doctor in D3 but forgot that I changed character and charged into a group of about 15 enemies on Nightmare. Died almost instantly, so at least it was painless.

I was in danger status in Resident Evil Outbreak File 1. I walked by a big picture window and it exploded. A zombie grabbed me and dragged me from the room... My jaw was on the floor I was so surprised.

Someone saw me in Deus EX Human Revolution.
