I got killed by a Scout spy-checking me with his scattergun and getting a crit; if it had been a normal shot, I wouldn't have died and would have managed to backstab the damn Medic before he ubered (probably his pet Heavy as well).
I'm not sure if it was the scout coming off the battlements, the soldier from the grate, or the ubered demoman in the courtyard, but they took out myself, my sentry, and our intelligence. 2fort is a harsh mistress.
Well i was playing DayZ, and I was being chased by about 20 zombies into the woods while unarmed. Eventually I said 'fuck it' and stopped, allowing myself to be pummeled to death by a circle of angry beings
Probably getting shot out of the air while diving in slow motion at some mobsters in the original Max Payne. On another note i'm only playing that game for the first time and it is absolutely fantastic.
Don't remember exactly how but I know that I died last in a multiplayer match in Starhawk.

Seems alright. I just got it yesterday so I haven't played a lot. It really just depends on your team and the other team. If you're team doesn't set up defenses, then the enemy can come right in.
I think I last got killed while trying to massacre the entire police force in GTAIV again, I forget. Either way, it was still pretty fun, lol. (*cough* In a...completely respectable sense, of course.)
Got shoot during a level in Max Payne 3, health is now all red and the game place me in Last Stand (the opportunity to kill however shot you with that critical shot). Problem was, I couldn't shoot back because dear ol' Maxey entered Last Stand BEHIND A FUCKING LIGHT POLE! A full AK-47 clip, ZERO hits (on Hardcore difficulty). I ragequited the game and went playing Trackmania.
I got shot by some Geth in Mass Effect after first talking to Liara (when she was behind that shield thing).
Didn't even realize my health was so low until my second death - first time was from a Carnage Rocket which insta-killed me through my shields so I didn't notice. Second time was due to regular old bullets.
I got ass raped by Shao Kahn in the last stage of MK9. Haven't played that game in a year and the campaign is still as fun as it was when I first beat it.
I constantly fell to my death trying to deal with some of the pretty frustrating platforming parts of Baldur's Gate: Dark Alliance. Who honestly thought that was a good idea to put into the game? At least there are a lot of save points, though.
