I was addicted to Dr. Robotnik's Mean Bean Machine, but really, who wasn't?
The original Tony Hawk was the next big one. My God, I must've rented it 10 times before I finally got it for my birthday, right when THPS 2 was being released on the PS1! Us N64 kids had to wait for the port.

I always associate H.E.R.O. with Pitfall! because my mum got both of them for me at Christmas one year. She bought H.E.R.O. and the dude said she could just have the Pitfall! cartridge for free because they'd lost the box for it and couldn't sell it like that! Bonus.
Not a particularly original choice, but Super Mario on the NES back in '86. The first video game I played with an actual goal and not just racking up a high score.
I've only had two, true gaming addictions. Lost Planet Online and NBA 2K7. I ranked top five in NBA 2K7 for many months. I played a lot of ball and I would bet anyone here in a game of 2K7 if I still had my 360.
Edit: Super Punch Out, Cool Boarders 3, Tony Hawk's Pro Skater 3 and Call of Duty would be runner ups.
I'll just do this in sort of cronilogical order.
Atari 2600: Plaque Attack
NES: MegaMan 3
Genesis: Sonic 2
N64: Goldeneye 007
PC Warcraft 1/Roller coster Tycoon/ Guildwars ( had about 3,150hours, which by MMO standards isn't a lot but still)
Now a days it's probably Rock Band/Guitar Hero cause a bunch of buddies play it so.
Not that I'm an extrovert but my socializing irl went bankrupt when Counterstrike Source came out. I've easily put in over 1000 hours with pubs, my clan, and some CAL. My longest gaming session was 13 hours. I only took bathroom breaks and my ass, pants, and chair were drenched with sweat.

The game that got me first addicted, I think, was Age of Empires or Total Annihilation. And then SimCity 3000. But that was ridiculous. I had lunch, played a bit of SimCity and then it was dinner time. It was like time got fast forward 16x. A similar experience was playing Civ 4. It was a time-consuming game. Most recent addiction was Call of Duty 4. Insane 2 months. There was a day that I played the game start to finish 2 times.
But the games that got me addicted for a longer period of time (from release date until now) were Freelancer (this game is awesome) and GTA: Vice City (Mr. Vercetti is the da Boss).

However, after consideration, I must admit that X-Wing on PC beats Goldeneye by a few years...

Well, now that I think about it Rollercoaster Tycoon was a big habit for me. I don't know which came first :D.
And no specific game but I've beat all the good Mega Man games (1-6, X-X6) atleast 3 times each, all upgrades and all hearts for every single playthrough, well for the X games atleast.
Then there's World of Warcraft, I think I played that game regularly from 2006 to about 4 months ago.
No seriously, I was addicted as hell to that game when it was on the old Macs in Elementary school. It also sprung up again in my AP Human Geography class my Freshman year of high school. Lots of good memories with that game.
